Name Change - Voting in Nassau County?
I received a postcard in the mail, telling me where to vote in November.
It has my maiden name on it. Does anyone know if you can change the name on a website or do you have to go somewhere in Mineola or do you bring your marriage certificate to the polling place?

You just have to fill out a form, but you have to either mail it in or bring to a local office. I just sent in my form for a change of address, since I still have to change my name.

Where did you get the form? I looked at the Board of Elections website and all I could find was a Voter Registration form. I couldn't find a Name Change Form.

These forms should be one in the same. I believe it has a place for a former name/address and your new info.

I got mine from Rock the Vote. Here is the link:

I checked and it is the same form for first time voters.
