Me too! So many things went wrong and I figured I should send it anyway.
I will need an extenstion too. My provisional expires in a year and I lost my job in June and still have not found one. 
So I don't have a class to film it with.
I think the scores come out at 5:00.
I am so nervous. I really think I failed. I am hoping mine was one of thiose that they never looked at and passed!
Well, there is that rumor.

listen to this fiasco!
When i was filming. my AP came in and interupted, announcements were made on the loud speaker, the tape ended just before i finished the "wrap up". then had it transfered and my sister picked it up, by the time i checked it found out they put it on a DVD, had to figure out how to transfer it onto a VHS. got that done, didnt go to work next day, mom was in hosptial, went to send it and realized i never got a tenured teacher to sign it, had to fax to AP to sign it for me, then mailed it OVERNIGHT, to barely make it there on time. (since of course i waited until the last min to film)
 so after all that it might be a miricle if i passed haha Kepp your fingers crossed!

I have a simliar story.
I had to send my express mail too! I had such a hard time transferrign that to a VHS.
I spent $145.00 on the video, $25.00 to transfer it to VHS, and $20.00 to ship it overnight.
I wen tthrough so much crap to get hat video done and after all that money, I better have passed.
I am worried though. I am really not confident that it was good enough. What was your lesson on?

you have to apply with the state to get your permanent. for mpermanent you need (some you might have from providional):
Passing grades on all tests (CST, video, LAST, ATS-W)
Master's degree
at least 2 full-time years in a teaching position teaching in the area you want the permanent certificate in
Child Abuse workshop
SAVE workshop
Fingerprints
go to www.nysed.gov
Go to teachers and then certification. Create an account on their on-line system called TEACH. Your provisional certificate and scores should be in the TEACH system and then you just apply for the perm. cert. online.
If your provisional or any other certificates doesn't expire before your wedding, i would apply for your permanent cert. AFTER your wedding with your new name. otherwise, it's $25.00 for a name change on your cert. it is a $100.00 for the perm. cert.

Wow, thanks...what if I send it and I am denied??? Do you think I have to send 100.00 again??? I may be short the two years of teaching. I was told some PT and subbing work counts towards it though.

you sohuldn't be denied as long as you meet the above requirements. they recommend a letter on district letter head detaling sub or part-time experience from the district.... they will apply that time to make up the 2 years. you usually have 5 years on the provisional, so if you got it right before they stop doing prvisional you have until 2008-2009.

I have met all of the other requirements. the masters, the tests, workshops, but the two years is still not complete.
Everytime I call them it is such a horror!
Do you reallys till need to prove the woskshops and some of the tests when I wouldn't be allowed to teach on my provisional wiothout it???
I am not sure if I can dig up all of that old stuff.
My license does expire 9/8 so I am a bit worried. Especially since I lost my job in June and have not recieved one call yet.

I wouldn't worry...It would take a horrific lesson to be denied a passing grade. Plus, I heard that they aren't all viewed....Someone I know (4 years ago) mailed in a blank cassette by accident and ending up passing anyway.
And, the state is phasing them out as a requirement for permanent certification...
I passd mine 3 years back.

Trust me they look at them....I did a great lesson and the woman who video taped me messed up the tape...there is a 4 minute block where the taped stoped and now I need to do it over again!

If you have the other requirements such as the workshops and fingerprinting, etc. etc. they have that on FILE. you do not need to re-submit those things for the permanent certification. create an account on the TEACH thingie on the site and you won't have to call them!
Also, get letters from the districts you worked for (part-time, subbing, etc.) if it all adds up to two years, you're good.
if it doesn't, you can apply for an extension on your provisional. i did this when i was working outside teaching for 3 years... i did not want it to lapse and be screwed and then have to meet the requirements under the new guidelines. you can apply for an extension on-line. HTH!
