Marriage License and Divorce...is proof needed????

We have yet to get our marriage license, and are supposed to do it this week, and my fi doesn't have the paper, which hasn't been sent by the lawyer. Does anyone know if they just ask questions on the form, or if you need proof of the divorce?

I looked into this for you and according to the NYS Dept. of Health, Vital Statistics Dept: Previous Marriages- Information regarding previous marriage must be furnished in the application for a marriage license. This includes whether the former spouses are living, and whether the applicants are divorced and, if so, when, where and aginst whom the divorce or divorces were granted. A certified copy of the Decree of Divorce or a Certificate of Dissolution of Marriage may be required by the clerk issuing the marriage license.
