Marriage License and Divorce...is proof needed????

We have yet to get our marriage license, and are supposed to do it this week, and my fi doesn't have the paper, which hasn't been sent by the lawyer. Does anyone know if they just ask questions on the form, or if you need proof of the divorce?

This is from the town of Hempstead's page (link below) You will need a certified copy of the Divorce paperwork. You can go to the site for more info, and even download the forms I think.

Both applicants must appear in person. Two forms of ID are required (one must be valid photo ID). Marriage license applicants whose previous marriage(s) ended in divorce, annulment or death of spouse, must provide proof of dissolution of all previous marriage(s) by presenting a certified copy of Divorce or Annulment or certified copy of Death Certificate for each previous marriage.
