RP from 05B - please help me with reception card info so I can order my invites!

I want to write this:

Adult Reception, Dinner and Dance
half past six in the evening
Jericho Terrace
Mineola, New York

My questions are:
1) do I capitalize the "h" in "half past..."
2) do I write out the address for JT?
